We bring you our weekly SonoSim Ultrasound Challenge! Can you give us the correct answer?

This 36-year-old female presents with intermittent severe a abdominal and pelvic pain for one day. It is diffuse and radiates to the mid abdomen. She has not had it before. When the pain becomes severe, she reports nausea and has had one episode of vomiting and no diarrhea or fever.
Vitals: T=37 C BP=98/57 mmHg=115 bpm RR=20 bpm O2 sat=98% on room air.
