Background: This is a 76-year-old male who presents after feeling and hearing a pop along his right anterior ankle while walking. He has difficulty dorsiflexing his right foot. Please evaluate his right anterior ankle with ultrasound.

👩‍⚕️ Scanning & Imaging Tips: Refractive Shadowing in Achilles Tendon Rupture

 

In this case of Achilles tendon rupture, the retracted part of the Achilles tendon has multiple shadows within it. That's typically what we'll see when a tendon is ruptured and retracted proximally. The proximal part of the tendon will have these refractive shadows. And basically, that is because as the Achilles, let's say, almost balls up proximally behind a rupture, it will cause refraction of the ultrasound beams. That's one of the artifacts to look for, which can help you diagnose that this is, in fact, a complete Achilles rupture.
